Online Sexual Violence and Sexual Harassment (SVSH) Incident Report Form

You may use this form to file a report of sexual harassment or sexual violence with the University instead of filing the report directly with the Title IX Officer. If you are unsure whether filing an online report is the best option for you or would like more information about your reporting options while remaining anonymous, you may speak with a confidential Victim Advocate at the Center for Advocacy, Resources and Education (CARE; 530.752.3299; or one of the other confidential resources at UC Davis, or place a call to the Harassment and Discrimination Assistance and Prevention Program’s (HDAPP’s) Anonymous Call Line at 530.752.2255. You may also want to consider the following prior to submitting an online sexual violence or sexual harassment incident report:

  • If you are in immediate danger call 911.
  • Confidentiality: The University will protect the privacy of individuals involved in a report under the UC Sexual Violence and Sexual Harassment Policy to the extent permitted by law and by University policy and procedures. However, reports filed through this online portal are not confidential. To effectively respond to the report, the Title IX Office may need to share some or all of this information with campus administrators or, in the case of an investigation, investigation participants, including named respondents and witnesses.
  • Responsible Employees: Completing and submitting an online report does not fulfill your responsibility to report suspected incidents of SVSH to the Title IX Office. Please call or email the Title IX Office or HDAPP directly to report an incident.
  • Please provide as much information as possible.  Submitting a report anonymously or with incomplete information will reduce the University’s ability to respond to the allegations.  All reports submitted via the online SVSH incident notification form that contain alleged behavior prohibited by the UC Sexual Violence and Sexual Harassment Policy will be forwarded to the Title IX Office.The Title IX Office will address the allegations to the maximum extent possible and based on the information submitted.  Reports alleging behaviors other than those prohibited by the SVSH policy will be forwarded to the appropriate campus office when possible.
  • Unless the reporting party is also the person who experienced the reported conduct, they will not be informed of any University action taken as a result of the report.
  • Submitting a report via the online SVSH incident report form is not the same as filing a report with the police department.  If you wish to file a report with the police, please contact the police department that has jurisdiction over the location where the incident occurred. A CARE advocate can help you determine which law enforcement agency to contact if you are not certain.

Complainant Information

The Complainant is the person who experienced the alleged conduct.

If you provide identifying information for the Complainant, the Title IX Office will notify them that an online report has been received, provide them with information about campus resources and reporting options, and, if they decide to file a report with the University, invite them to contact HDAPP or the Title IX Office directly.
